Water spray not operational — stop aggregate handling immediately. Condition 1f.iii.
Dust must not be visible more than 15 min in any 1-hr period — Condition 1e
Excess dust — Condition 1e. Apply controls immediately and document below.
Dust present but controlled — document measures below.
If observed, Method 9 opacity observation required — Conditions 2c/2d
Visible emissions — open Opacity Observation form. Method 9 required before resuming normal operations. Conditions 2c/2d.
Breakdown — if excess emissions possible: call (505) 224-6977 AND fax (505) 768-1977 within 2 hours. Open Equipment Breakdown Log. Condition 5d.
Both 1634-RV1 and 1634-2AR revision letter must be posted visibly — Condition 9
Permits not posted — both documents must be displayed at the plant site before operations begin. Condition 9.
